Eligible Medicare beneficiaries can access covered digital MSK care without copays or coinsurance under the model.
A Funded Care Pathway
myHealthTrack receives payment directly through the applicable CMS ACCESS reimbursement structure for qualifying MSK care, rather than requiring the ACO to fund that covered pathway.
Physician Co-Management Opportunities
