That longitudinal data gives clinicians visibility into how patients are responding between visits, enabling timely check-ins, plan adjustments, and early intervention before issues escalate into avoidable utilization. Patients see their own progress reflected back to them as well, reinforcing motivation and accountability throughout recovery.

What is MyHealthTrack?

myHealthTrack is a daily health program for patients like you living with lasting pain — in your back, neck, shoulder, hip, knee, or anywhere else. It's offered through a new Medicare program called ACCESS and it covers you at no cost for up to a year if you have Original Medicare Parts A and B.

What you get

  • Daily lessons built for your condition. Movement and exercise, nutrition, sleep, mindset, and understanding your pain — designed by clinical experts and matched to where it hurts and what you want to get back to doing. Like having a team of coaches in your pocket.
  • A physician who stays involved. Through CareCompass, a doctor keeps an eye on how you're doing along the way.
  • Regular check-ins. Short questionnaires about your progress, so we can see what's working and adjust.
  • Your own doctors kept in the loop. With your permission, we send updates to your primary care provider or care team.

Who can join?

You may be eligible if you:

  • Have Original Medicare Part A and Part B, with Medicare as your main insurance
  • Live in an area myHealthTrack serves
  • Have muscle or joint pain that has been going on for more than 90 days
  • Are not in hospice care or the PACE program

You don't need a referral. You can sign up yourself.

If you have a Medicare Advantage plan, you aren't able to use this care option — but your plan may offer something similar, so it's worth calling them.

There's no cost to you.

Medicare covers the program, and myHealthTrack does not collect copays or coinsurance. You won't get a bill for myHealthTrack, and there's no cost to find out whether you're eligible.

Care you get from your other doctors is billed the way it always has been.

What happens after you sign up

  1. We check your eligibility

    You'll give us a few details and we'll confirm with Medicare that you're able to join. This takes just a few minutes.

  2. We match you to a track based on your condition

    You provide your existing diagnosis, or answer questions about your condition and health history. We recommend an appropriate track.

  3. Your care starts

    You'll get your digital care plan, which you can access via the web or our mobile app on any device.

Good to know

  • Enrolling is your choice. Your Medicare benefits, coverage, and rights stay exactly the same whether you join or not.
  • You can leave any time after your first 90 days. You can stop, or move to a different organization, and we'll make it easy.
  • You keep your own doctors. CareCompass adds to your care. You can still see any provider who accepts Medicare.
  • One CMS ACCESS organization at a time for muscle and joint pain. If you have another qualifying condition, you can work with a different organization for that one.
  • Some people are placed in a control group. To study how well this works, Medicare randomly assigns some people to a group that can't enroll for 12 months. It's not based on you or your health, and it doesn't change your Medicare coverage in any way.
